SoulFight Brazilian Jiu Jitsu – Liability Waiver & Media Release

The student acknowledges that they have voluntarily agreed to participate in grappling/martial arts practice and instruction at SoulFight Brazilian Jiu Jitsu, located at 20 Main St, Ashland, MA 01721.

The student understands that this type of sport is inherently dangerous, regardless of the level of supervision. Injuries may occur due to various physical situations, including but not limited to a slippery floor surface, an overzealous partner, or a combination of unpredictable circumstances. The student recognizes that voluntary participation in this activity exposes them to risk of injury and that prolonged engagement increases the likelihood of such injuries.

THE STUDENT IS VOLUNTARILY PARTICIPATING IN THESE ACTIVITIES WITH FULL KNOWLEDGE OF THE RISKS INVOLVED AND HEREBY AGREES TO ACCEPT ALL RISKS OF INJURY OR DEATH.

As consideration for being permitted to participate in these activities and use the facilities provided by SoulFight Brazilian Jiu Jitsu, the student agrees not to make a demand, claim against, or sue SoulFight Brazilian Jiu Jitsu, its employees, or authorized agents for any injury or damage resulting from negligence or other acts, however caused, arising from participation in martial arts/grappling. The student releases SoulFight Brazilian Jiu Jitsu, its employees, and authorized agents from all actions, claims, and demands that they now have or may hereafter have for injury or damage resulting from participation.

The student grants permission to SoulFight Brazilian Jiu Jitsu to take photographs and videos during classes, events, and competitions for promotional purposes, including use on social media platforms, websites, and marketing materials. If the participant does not consent, they must inform staff in writing.

SoulFight Brazilian Jiu Jitsu is not responsible for lost, stolen, or damaged personal items brought onto the premises.

Parents/guardians acknowledge full responsibility for their child’s behavior while on the premises. Should a child damage any property, the parent/guardian agrees to cover repair or replacement costs.
